First Visit

Initial Consultation

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Frequently, surgery of low complexity can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment. This may be at the office or one of four hospitals that Dr. Yapa is presently an attending surgeon.

For treatment requiring hospitalization, scheduling can be accomplished after consultation and necessary diagnostic work-up. Dr. Yapa operates at Henry Ford-West Bloomfield, Mount Clemes Genreal Hospital, Sinai-Grace Hospital-Detroit and William Beaumont Hospital -Royal Oak.

You may pre-register with us online by clicking here and filling out our online registration and health history forms. This will save a great deal of time and may avoid unnecessary delay in treatment.

Please assist us by providing the following information at the time of your consultation:

Your surgical referral slip and any X-rays if applicable
A list of medications you are presently taking along with allergies
If you have medical or dental insurance, please bring proof of both as they may reduce your portion of the applicable fees. When you make your appointment, giving our receptionist this information will save time and allow us to help you process any claims.

IMPORTANT:All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

A pre-operative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. Please have nothing to eat or drink 8 hours prior to your surgery. Wear loose clothing and avoid remove any nail-polish. You will also need an adult present during the time of the surgery who will drive you home.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.

Jaw Surgery Patients

Please bring recent models (no older than six weeks), radiographs and photographs necessary to diagnose the patient at the first consultation. If you do not have initial records, we will take them at the initial consultation. (There will be an additional fee for these records)
